Important: You cannot use one form of identification for both name and address. For example, if you provide your driving licence as proof of name, you must provide a separate document — such as a utility bill — as proof of address.
Proof of Name
One document required
- Current signed passport
- Original birth certificate (UK, issued within 12 months of birth)
- EEA member state identity card
- Current UK or EEA photocard driving licence
- Full old-style driving licence
- Photographic CIS4 card (self-employed construction)
- Benefit book or original notification letter from Benefits Agency
- Firearms or shotgun certificate
- Home Office residence permit (EEA nationals)
- National identity card with photograph
Proof of Address
One document required
- Utility bill (gas, electric, satellite TV, landline) — within 3 months
- Council tax bill for the current year
- Current UK driving licence (if not used for name)
- Bank, building society or credit union statement — within 3 months
- Original mortgage statement from the last full year
- Solicitor's letter confirming recent house purchase — within 3 months
- Council or housing association rent card or tenancy agreement
- Benefits Agency notification letter (if not used for name)
- HMRC self-assessment letter or tax demand (current financial year)
- Electoral Register entry
- NHS Medical card or GP registration confirmation
